Library Card
Use your library card to check out a wide variety of materials such as books, magazines, DVDs, CDs, audiobooks, eMedia and more. A library card also grants you access to databases and entertainment.
Fill out the online application and then stop by the Perry Public Library and upload your photo identification, such as a driver's license, showing your name and current address. If the photo ID does not include the current address, other documentation must be provided. If you apply online, please allow 24-48 hours for processing your request. Your card will be mailed to you if over 18 and applying online.
Minor children must have a parent or guardian sign their application form in person at the library. No online cards can be issued for children under the age of 18.
Full library privileges will be granted upon receipt of a library card following a probationary period.
Library cards are issued for a period of one year. Upon expiration, the card may be renewed after registration information is verified, and charges, if any, are paid.
Initial library cards are free, but there is a charge for a replacement card. The current charge is $4.
Cards are to be used by the card holder only.
