Perry Public Library Card

Use your library card to check out a wide variety of materials such as books, magazines, DVDs, CDs, audiobooks, eMedia and more. A library card also grants you access to databases and entertainment.

Adults: Fill out the online application to the left for the person applying for a library card. Provide proof of identity and current address by uploading a government-issued photo identification for the applicant. The physical library card will be mailed to the address provided in the application.

Youth (Parent applying): Fill out the online application to the left for the person applying for a library card. Provide proof of identity and current address by uploading a government-issued photo identification of the parent or guardian. Please indicate the rating of movies allowed to be checked out by the minor child. The physical library card will be mailed to the address provided in the application.

Youth (14 years to 17 years of age): Fill out the online application to the left for the person applying for a library card. Provide proof of identity and current address by uploading a government-issued photo identification of the applicant. (Example: State-issued Instruction Permit). Only G-Rated video materials are allowed to be checked out without in-person parental written consent. The physical library card will be mailed to the address provided in the application.

Full library privileges will be granted upon receipt of a library card following a probationary period.

Library cards are issued for a period of one year.  Upon expiration, the card may be renewed after registration information is verified, and charges, if any, are paid.

Initial library cards are free, but there is a charge for a replacement card.  The current charge is $4.
